Intake/Interview Form 13614-C
Before preparing your tax return, you must complete an Intake/Interview Form that provides important information necessary to prepare your tax return. The form is available at our tax site and is completed prior to seeing a tax preparer.
To save time on your appointment day, access the form below, complete it and bring it to your appointment. The form is a "fillable PDF". If you have a PDF application on your computer, you can complete the form online, print it, and bring it with you. Alternatively, you can print the blank form, complete it by hand and bring it to your appointment. Click the button below.
